Clinical History

57-year-old man with progressive left hemiparesis and headaches. Neuroimaging shows a large multicystic mass in the right hemisphere.

Case Discussion

Gliosarcoma is a variant of glioblastoma, a malignant glial neoplasm of the brain. Gliosarcoma is characterized by a malignant mesenchymal component in addition to areas of more typical glioblastoma. These components may resemble fibrosarcoma with long packed bundles of spindle cells or undifferentiated pleomorphic sarcoma. There may be mesenchymal differentiation with heterologous components such as bone, cartilage, muscle, etc. Staining for GFAP is usually negative while stains for collagen and reticulin are positive.
