This breast carcinoma has pushing borders. The glands are microcystic, solid, and microtubular. There are luminal secretions within the tubular glands. The stroma shows central sclerosis. The cells are polygonal with granular to foamy clear cytoplasm. The nuclear grade is low. A core-needle biopsy site is also seen. These features are seen in the exceptionally rare mammary secretory carcinoma, which has a low-grade clinical course compared to breast carcinoma of no special type. Secretory carcinoma is associated with a characteristic balanced translocation – t(12:15), that creates a ETV6-NTRK3 gene fusion.
