Adenocarcinoma in situ (AIS) is a lesion of the cervical glands that carries significant risk of invasive adenocarcinoma.This case is a variant of AIS, the stratified‐type (also referred to as a stratified mucin‐production intraepithelial lesion or SMILE) consists of stratified epithelium with cells containing mucin. It can be difficult to distinguish from reactive squamous metaplasia, and the diagnosis is made based on the degree of cytologic atypia throughout the neoplastic cells.
