49 year-old female patient with a unilateral ovarian tumour.
Sections show a well demarcated lesion composed of sheets of round-polygonal cells with abundant eosinophilic cytoplasm. Occasional bizarre atypia is noted. An important histological finding is the presence of Reinke crystals: rectangular, eosinophilic intracytoplasmic crystals, which are characteristic and necessary for distinguishing from other steroid cell tumours.
The lesion showed immunoreactivity for inhibin, calretinin, and MART-1 (not shown), which are supportive of this diagnosis.
Signs and symptoms related to excess androgen are commonly encountered in these patients. Prognosis is excellent, and unilateral salpingo-oophorectomy is generally sufficient for management.
