49 year-old female with bilateral ovarian masses. Intraoperatively, abnormal looking appendix and abdominal wall nodules were identified.
Appendiceal adenocarcinomas commonly metastasize to the ovary. This is a case of the less common variant of adenocarcinoma ex goblet cell carcinoid, a variant of appendiceal cancer with neuroendocrine differentiation. The slide shows ovarian tissue infiltrated by nests and small glands of epithelial cells containing blue mucin and having goblet-like appearance. The appendix (not shown here) was infiltrated by the same tumor cells along with focal area of conventional adenocarcinoma (poorly differentiated and signet cell carcinoma). The tumor cells are positive for CK20, CDX2, chromogranin and synaptophysin.
