Clinical History

This specimen is a right preauricular excision from a 58 year-old female. The lesion was described as a raised nodule measuring 4.5 cm, with a tan-brown hemorrhagic area. The patient was diagnosed with Brooke-Spiegler syndrome, a condition that involves multiple skin adnexal tumours.

Case Discussion

Cylindroma is a benign cutaneous adnexal tumour that shows ductal differentiation. In this case, the diagnosis is mixed adnexal tumour.

On histology, cylindroma presents as islands of basaloid epithelial cells that are arranged in a jigsaw pattern, surrounded by a thick basement membrane. Hyalinized areas can also be seen in the islands.

Although not found on this slide, trichoepithelioma and spiradenoma were also found in a separate biopsy, which is typical for Brooke-Spiegler syndrome.
