Case Discussion

Uterine adenosarcoma is a relatively rare tumor of the gynecological tract that comprises both benign epithelial elements and malignant stromal components. Morphologically, this lesion is characterized by a broad "leaflike" architectural pattern, analogous to that of phyllodes tumors of the breast. For instance, periglandular cuffing of the stromal cells around the compressed or cystically dilated glands is frequently observed. The epithelial component usually shows endometrioid type morphology with possible squamous or mucinous metaplasia, whereas the stromal component is typically a low-grade spindle cell sarcoma. Important prognostic factors to assess in these tumors include the presence of myometrial invasion and sarcomatous overgrowth.
