Case Discussion

High grade papillary urothelial carcinoma is a lesion in the spectrum of papillary urothelial neoplasia. The high grade version shows a disorderly appearance of the urothelium with easily recognizable architectural and cytological abnormalities.

Some of the specific microscopic findings one would see include cytological pleomorphism ranging from moderate to marked, clumped chromatin, prominent nucleoli, mitotic figures at all levels of urothelium, variation of urothelial thickness, and papillae that are frequently fused.

This case shows invasion into the lamina propria.
