61 year old female with a past history of hypertension was found to have a right posterior orbital tumour suspicious for malignancy and was treated with a right orbitectomy.
There are a wide range of diseases that can be associated with inflammation of the orbit. In this case, the orbital soft tissue shows extensive non-necrotizing granulomatous inflammation with some granulomas that contain foreign material. There is also a mild associated lymphocytic inflammation. Special staining for micro-organisms (Gram for bacteria, AFB for mycobacteria, and PAS-D for fungi) are negative.
The differential diagnosis for this lesion includes granulomatous orbital inflammation reacting to foreign material, orbital involvement by sarcoidosis, and idiopathic orbital granulomatous inflammation (ie. 'orbital pseudotumour'). Although microbial stains were negative, this does not entirely exclude an infectious etiology.
