Chronic inflammatory injury to the vulva often presents as a white scaly plaque-like lesion. In this case, the features are of scarring and atrophy. Lichen sclerosus (characterized by atrophy and dense dermal collagen) can occur side-by-side with lichen simplex chronicus (characterized by hyperkeratosis, dermal thickening, and dermal inflammation). This case has a mixture of features.
