Clinical History

12 year old male with recurrent nose bleeds over the last 2 weeks. Nasal cavity mass was excised.

Case Discussion

Lobular capillary hemangioma is a common benign oral lesion seen in pregnant women and pediatric populations. It has been linked to poor oral hygiene and oral trauma. The other common name for this entity, pyogenic granuloma, is a misnomer because it is non-infectious and does contain any granulomatous features.

Grossly, early lesions are hemorrhagic with or without focal ulcerations, and later on become fibrotic. This section shows a polypoid vascular lesion containing various sized blood engorged vessels with benign endothelial lining; arranged in a lobular architecture. The overlying epithelium is hyperkeratotic with hypergranulosis, focal ulcerations with scattered inflammatory infiltration. The lesion is positive for vascular markers CD34, CD31, factor VIII and SMA.
